Anyone know the Diagnosis Code for Psych Eval?

I have United Health Care Choice Plus and I'm having MAJOR problems getting a diagnosis code in order to submit my claim for reimbursement. The doctor saw me at my surgeon's office and the receipt didn't have the diagnosis coe or the service provider code. I was able to get the service provider code, however can't seem to get anyone to get me the diagnosis code. The doctor keeps telling me there isn't one because I don't have an illness. The insurance co says they can't process the claim without it. I'M STUCK IN LIMBO and I could use my $150..

Can anyone please help me???

I have the same insurance and had written down all the inform,because the psychologist ask me to call first to see if its covered,but I cant find the info.The only notes I found was to bill to UNited Health Behavioral.Call customer service for UHC and see if they can give it to you.Who did your psyc evaluation ask them what code they use to bill.Sorry I couldn't help further.

They would probably have to bill it as depression or anxiety or something in order for insurance to pay for it.  I don't think there is a diagnostic code for a WLS psych eval.

V72.83 (Other specified pre-operative examination - pre-operatiave exam unspecified).
