Highmark Insurance Question

Does anyone on here know the medical necessities/requirements for HIghmark BCBS to cover panni surgery?  I know some have specific requirements.  Not sure about Highmark.

I have Highmark BCBS - but not sure about this.  When I needed to find out what was required for Lap Band, I called & they faxed me the policy with the specifics about WLS.  Just call & they will tell you.
