Personal trainer required by insurance, ever??

Has any of your insurances REQUIRED you to work out with a personal trainer? Exercise therapy is listed as a requirement but I am thinking I can just tell my doctor what I am doing and still be fine in that department.

I did pay a trainer to work out with me for the first and second mth just 1x each mth but I really don't want to keep paying right now with everything else if I don't have to.

Advice?
