Aetna & weight history

I have Aetna HMO and I did not go to the doctor in 2007 or 2008, and 2006 was all prenatal appointments (but am over 40 BMI at the first one, 10 weeks pregnant). The policy states:

Presence of severe obesity that has persisted for at least the last 2 years, defined as any of the following:

  1. Body mass index (BMI)* exceeding 40
I emailed someone that deals with the financial stuff and was told I needed records from 2008 and 2009 or dated pictures. We never use the time/date stamp feature on our camera but I can think of at least 2 pictures that I'm in, I just have to find them.

Would this possibly be enough or will I have to wait until next year? (I have not done a 3 or 6 month diet yet anyway, my policy says "Must have surgical consult at beginning and end of program" and when I called to try setting up an appointment, I was told I needed to do the seminar first, which I did a few days after the phone call.

I also have Aetna and after my surgeon submitted the insurance approval they came back stating that they need a 5 year weight history and the 3 month consecutive diet regiman or 6 months combined diet.

All this conflicting information is really difficult to know what the hell they really need.

I also have Aetna: If you do the 3 month here goes.. First attend seminar then consult w/surgeon. start weight loss physic an documented. Make apt for a nutritional evaluation w/a nutritionist or dietitian if you are a diabetic a dietitian at local hospitals may cover under plan.. continue with the 3 months of documented weight loss mg mt w/ PCP. make sure Dr is very detailed in notes. make sure app ts are exactly 30 days apart. you will see your PCP for 4 visits in total.. then make sure you get a psych evaluation find one that is familiar with obesity and ls. then at the end of your documented weight loss go back to surgeon office. Your PCp will need to write a medical neccesity letter elaborating on your weightloss attempts and or comorbodites. Also a medical clearance letter stating that pcp believes this will enhance your way of life and quality of life. The surgeon office will then submit paperwork to AEtna.. Oh yeah they need 2 years of weight history also.. You can review some of the sample letters on some surgeons website. ie suncoastbariatrics.com. you can see the samples to help shed some light.. Look up you have come to far to let Aetna distract you know.
