Confused about BCBS expectations

I have just recently started working at a hospital as a registered nurse and now have BCBS of Florida insurance. They require a 5 year history of obesity......sooo can I just contact the docs from the past 5 years and request the parts of the chart that show my weigh in? Each time I was heavier and heavier. Also they say you need the approval from your PCP......I just moved to Pensacola and have different insurance than my old job so I will have a new PCP...does it matter that I have a new PCP? Does the insurance care how long I have been with this PCP?? I am just confused and really want to start the process.......HELP!! Thanks

Okay.  For five year history, you're correct---just get copies of your medical records from whatever doctor visits you've had in the last 5 years---they should have your weight recorded on them and you can just copy and highlight the weight part to give to your insurance coordinator. 

As far as the approval----I had to meet with my doctor once a month for 6 months for a supervised diet requirement and at the end of all of that, my doctor put on there that he recommended me for bariatric surgery.  Some insurances require a letter from the doctor so you might want to look more into that.

My first appointment for the supervised diet with my PCP was my first visit with that doctor.  They don't seem to care how long I've seen that doctor as long as it's a doctor.

They only want to see a history.  You can get medical records from different doctors if you need to and it shouldn't matter. 

When I started my process with BCBS of New Jersey last summer my PCP wasn't really supportive of wls so I change doctors.  You'll want to be with someone you are really comfortable with.

I agree with JENBBee!!! They ask for the five yr medical records, 6 month supervised physician diet (time fram seeing the doc does not matter) its the focus on the requirements and the letter from the supervising physician recommending wls. Note: keep the monthly visits consecutive, if you miss a month...you run the chance of a denial or bcbsfl asking you to restart the supervised requirements.
