Does anyone know anything about a Bariatric Eval Form for Tricare? John Hopkins is wanting one to send for approval. I am at the end of all the testing and now they throw this on me. What gives? You would think they had this on their list of things to do before surgery but nooo....

Never heard of this before... I would suggest calling Tricare and asking them what this is.

I have never heard of this either. Like Wendy said, I would just call them.
