Guardian Insurance and Terror

I am terrified of not getting approved by my insurance, which is a combo of Guardian and Independece Blue Cross/Blue Sheild with Guardian being primary.

Anyone have experience with them? 

Are you the patient?  Is Guardian primary through your employer?  and the BCBS being though your spouse?
