I found out on Monday I was approved.  I'm currently waiting for a surgery date.

The Weight Management Center at my surgeon's office offers a 6 month diet plan with  6 nutrition classes.  I logged in everything I ate for six months on  www.thedailyplate.com and turned it in to them monthly.   They kept track of my weight, kept notes on my progress and kept copies of my food log.  At the end of the six months,  they turned the notes in to BCBS with the rest of my file.   

You could do your six month diet with your PCP.  Just go in once a month for six months to have your weight checked and have the doctor keep detailed notes of your progress including all your co-morbidities.

It sounds like a pain but it's really not so bad.  Start your diet tomorrow and log everything you eat.  Six months will go by quickly.  I started my diet last July and it took 10 months from beginning to insurance approval.

Where you able to print the policy manual from the link I sent you?  What state are you in?
