Insurance and appeal letters

Hello everyone I know I am jumping ahead but I would like to know what would go in the letter to appeal a procedure? I have been approved before but have never made it past the physic eval. I would like to go to my first appointment with my letter in hand to add to everything else so that in the event that I am denied I will already have that on file, better than that so that I won't be denied.
Also, I know that my insurance (Pacificare POS HMO, San Diego, CA) has said that it depends on what my doctor says. Well I know my doctor and I have talked this out over the last 6-8 years so he knows what's up with me and weight loss. I have always made sure that I went to the doctor everytime I tried a new diet. But anyways I just wanted to hear from people that have had experience in this regards.

There are many free websites out there that can help you with what your appeals letter should contain.  I don't have the links accessable right now but if you use the words insurnace appeals I am sure you will get some insite.
