...but I wasn't insured 5 years ago (in school)  :-/ I qualified as early as 7-8 years ago, but I'm not sure there's paper trail to document it..... I have United Health Care EPO and their requirements include a 5 year history - anyone know if there's a workaround for that?

I am in a similar situation, however, I have many views but nearly no responses to the matter. Please keep me informed if you have better information. Good luck!

I've seen some suggestions in the past for this situation.

a.) Submit photographs from the missing years.

b.) Submit medical notes from another doctor (i.e. obgyn) for weigh-ins from the missing years.

c.) Submit weigh-in documentation from a gym, nutritionist, or any other weight loss program that you may have utilized.

I hope these suggestions help!

You just need a medical record from any doc who may have taken your weight.  When you were in school, didn't you ever go to the campus clinic for a sore throat or something?????

Hey there - I have UHC EPO as well and yes they are asking fir the 5 year weight history. I am going to submit my paperwork to the surgeon next month and I will let you know how it goes. All you need to show is a weigh in and nothing else. If you went to the hospital that may have weighed you, you can used that as well. Also is UHC asking you do a 6 month supervised diet. This is the last step for me. I am currently going on my 3rd month so I would like to hear from you.

Are you ladies saying UHC EPO or PPO. I have PPO and I am wondering they are going to ask me the same thing. The WLS center said that because of my BMI alone, that may help me. When I called and spoke to a rep. I was told that they may require some comorbidities with submission. I might make another call and ask that question again.. and see what response I get.

I am also going through the same thing.  I received my 1st denial yesterday. (of course I had to call them to find that out)  I have 1009, and 2008 covered from the 6 month weight loss program they asked for.  i started that in nov. 2008.  I was also uninsured before that so i never went to the doct.  If i got sick i did alot of that "does anyone have any antibiotics at home".   I am desperately searching to be sure that i don't have any documents that i forgot about.  If i don't im not sure where to go from there.  This is the only thing holding me back.  I have pics from like 1998 but the insurance comp. told me they would not look at them.
Please let me know what you find out
Thanks

Hi ya"ll - I submitted my 5 year weight history to the surgeons office and they said it was perfect. I had started a weight program 5 years ago with Transformation. I went to them and they still had my chart. I was extremely excited. They made copies and that is what I submitted along with my PcP weigh ins for thast couple of years. Well it was okay at my surgeons office.

Sunshine73, I have EPO... but from what I gather, specific levels of coverage (for both EPO and PPO under UHC) vary a bit depending on your employer. It also pays to ask and ask again (I've gone through half a dozen different reps and gotten varying opinions...) if you don't find them super helpul.

As for the weigh-in thing, do any of you ladies know if they really need 5 yearS of records, or if one single weigh-in from 5+ years ago is sufficient? As I mentioned in the first post, I didn't go to the doctor often from about 2001-2007 so those years have spotty records. I *did* get one medical record from 2003 with a BMI of 42 :D ..and am really hoping that will be sufficient. I am waiting on some of my childhood records (I was at least in the high 30's in my teens) and will then submit for approval.
