Aetna Paperwork Requirements

I'm finishing the 3 month Multidisciplinary program with aetna pos II. What else or did i forget anything? I'm building my packet with everything thats need  to avoid any delays when submited.
3mo dr visit-done(one visit left)
3 Nut vist
PSYC eval-4/1
Pre op nut visit 3/30
2 Year weight history starting from 06
Medical letter from pcp 4/3

I think that's all of it, Ash. Dr. S's office will also write a letter of medical necessity (which will be Teresa's first excuse for not getting the entire package out the door the day you're done!).

I just got off the phone with Tracy because, believe it or not, after the fun of insurance, there is still MORE paperwork and tests to be done pre-op!

Okay cool Jill. I just want to have everything submitted at the same time and not with any delays if possible. I think i will try and write the letter of medical necessity and have my doctor review and sign(wish i had a sample to guide me).

The never ending trail of paperwork. After all this i should have enough to right a book(LOL). Thanks for all the info.

I was approved on the 3 month multidisciplinary program by Aetna.  In addition to what you had, I also need to meet the requirements of exercise and behavior modification.  Make sure that your PCP has included this information in her office notes from each of your visits.  My PCP wrote in her notes that I have met with an exercise specialist and that I was walking xx amount of tiimes per week, etc.  For behavior modification, she noted that I had joined an online support group (OH) and a local support group.
