Thank you so much for posting this information! I have BCBS Anthem of Ca also. My surgeon's office submitted my request for VSG last week and I am expecting a denial. The info you posted is a great help! Congratualations on your approval!

Dept of Managed Care in CA overturns approx 95% of denied VSG and Ds surgeries so I would appeal to them if you need the address and etc to appeal to them I have that information

I will use this. Is your bmi over 50...because they are saying that they will cover it in a 2 part for the DS if my bmi is 50+. So why is it investigational still??? ugh.

Highest weight- 262lbs
Lap band- Aug 2006 - 254lbs.
Lowest w/band 214lbs. .
Gained up to 271 due to the band
Got DS revision April 2010!! Current 145lbs
At 5'8 my goal was 160lbs but I surpassed that with the DS!!!!!!
