Keystone Mercy

Does anyone know what Keystone Mercy (Pennsylvania Medicaid) requires in order to approve WLS (specifics with regard to diet/weight history is what I'm looking for?) What other appointments are required (nutritionist, psych etc.)

Google has turned up nothing so far. :(
