Sample appeal letters?

I need to appeal the denial by Tricare. Is there anyone who has had an appeal get approved? If so what did you put in your letter? I understand I will have to tailor my appeal to my specific situation. The basic wording is what I am looking for.

I just sent you a PM with the appeal letter I sent.  I was originally denied, and after sending an appeal was approved after 4 days.
