Appeal letter help....

I was told by my insurance company that everything was okay and I was approved for DS. Then last week they tell me I am being denied because I do not have a 5 year history of a BMI of 40 or greater. I got the letter in the mail Saturday. I do have co morbids, high cholesterol and sleep apnea.   I would like to know if any one else has been through this? I need to write a letter of appeal and would like to know if anyone has one I can use as a guideline to start.   Thank you.
