Peer to peer...surgeon or PCP???

I found out yesterday that my insurance United Healthcare is going to deny me because I have not met the condition of havnig a BMI of 40 or more for the last 5 years.  The 39.8 and 38.9 are not making it...  My question is if I have a peer to peer should I have my surgeon call for this or my PCP? 

I do believe it's the surgeon, not your PCP.  Ask your surgeon's office about it.
