Help with needing to find insurance that will cover WLS for my daughter

I know a lot of people talk about trying to get insurance that might cover WLS. My experience over the years as an overweight, high blood pressure, etc person, is that no one would approve me for any kind of insurance. When my coverage become unaffordable and I started shopping around, no one would insure me except the high risk pool which is BCBS of Texas. They exclude WLS even if medically necessary. So I self-payed.

To make a long story longer, my daughter is wanting the sleeve. She has Scott and White health plan. Scott and White is a huge  hospital where she works but they exclude WLS.

Has anyone had any success finding an insurance company that would cover an overweight patient with HBP and cover the sleeve? Especially one that won't make you jump through hoops? Not too much to ask right? HELP???

Generally speaking, an individual plan will not cover WLS.  Best option would be for her to find a new job.
