Health Mark Insurance

I have had a consultation with my doctor and just have to get my pscyhe evaluation in to her.  But has anyone dealt with Health Mark Insurance?  I think it's based in Indiana.  I was just wondering what kind of requirements and stuff they have for the surgery.  I tried to call but they are closed (of course) and was wondering if anyone had gotten approved thru them.  Thanks!
