5 year medical records
I have United Healthcare Choice Plus. I have met all requirements, except the 5 year history. I cannot seem to get my physicians to come up with the weights in my medical records from the visits I have made in the last 5 years. Does anyone have some advice on getting the needed information? I dont want to send for approval until I have all the required information.
Also, does anyone know how long it will take for my approval once submitted?

Hi Breanna--first, I agree--don't submit to your insurance until you have everything. Good idea.
Does United HCP require that your past five years of med recs have your weight listed? Can that weight be determined by other means (i.e. Weigh****chers, etc.) or does it have to be via your physician? What are they trying to determine?....if you were morbidly obese for the past 5 years? Comorbidities? Or just wanting five years of med recs?
As far as how long it will take, your Plan Summary will determine that. You can call the customer service number & ask them what your specific plan summary denotes as the amount of time they have to respond once the request is rcv'd in their office...they should be able to tell you.
By the way, I live in Franklin, TN....I am just finishing my 6 month supervised diet & xrcize required & the insurance has approved e'thing else...they just need my last visit & I "should" be done....I am having Lap RNY with Dr. Houston at Centennial--hopefully in Feb or early March.

I did not have a weight from all five years. I sent weights up to 8 years old and wrote the insurance company a letter saying that I had not been to the doctor every year for the past five years, but that they could see by previous years weights that I have been obese that whole time. I have BCBS...my information was sent in by the Dr. on January 2 (Friday) and they called me at lunch time January 5 (Monday) to tell me I was approved and schedule my surgery date. Good luck!
I also have UHC and I just got a call from my surgeon's office saying that my insurance company has requested a 5 year history from my doctor. The problem is that my doctor passed away suddenly last month, and his office has basically shut down. They won't even talk to you on the phone. They hold administrative hours 2 days a week when you can go in and request your medical records, so I'm going to try that next week and just hope that there are enough legible notes to satisfy the requirement because there doesn't seem to be anyone in the office who can write a letter. My surgery date is 3 weeks from today. I've been going through this process for almost a year now, so the thought of possibly delaying my date is excrutiating.
I have sent to 12 doctors and hospitals now and I have got weights for 2006 only. I cannot beleive this. I am going to try the picture thing and send the records I have with the notes saying I was obese and cross my fingers. I did my psych visit today.....good news, I am still sain!!!! Amazing. SO I am almost ready to send to the insurance company. thanks to everyone for the response.

I don't know if this will help but I actually went to my clinic and filled out a paper and requested all my records. I had to pay a small fee but I thought it was a good idea to have my records for the future anyway. I pulled out a sheet from one visit for each of the past 5 years.
If you have different Dr.s go to them all. You have the right to have your records and It is worth a little leg work.

I have the same health insurance plan as you do, and I am going through the same thing. What I've done is go back to any and all doctors that I've used in the past 5 years and requested medical records. I have only had insurance the past month, so I've had to contact Planned Parenthood to get copies of my ob/gyn records, any urgent care centers i've visited, and I am attaching dated pictures with a letter explaning the situation and a chart of my estimated weights for the past 5 years. Another good thing to do is visit your regular doctor, and have he/she take an official record of your medical history, as told by you and any paperwork you may have, and make a copy of it for when you submit to insurance.
