Which surgery? DS, VSG, RNY, Lapband? You should be able to get your benefits booklet from your HR department.  Once you find out if they cover WLS, just follow all their criteria and you shouldn't have a problem. Make sure you document everyone you talk to at the insurance company/ medical group(if you have a HMO)/ doctor's office, etc.... Best of luck to you.

Hey there... are you living in New York?  I have Empire BCBS (PPO) and it APPEARS they will cover it, but I've also heard stories from a few years ago that were nightmarish at best.

This is what I found on their website and it's the most recent update available.

I'm trying to get the DS done, which usually has more insurance issues as many companies still consider it investigational (even though it's not.)  I'll let you know how things turn out with them...
