Unless you are in CA, you are likely not going to get around the 6-month diet.
Doctors can be out-of-state. Find out from you company's HR department where to find out which doctors are considered in-network for your insurance. I have BCBS of NJ, but I live in VA and my surgeon is in MD and will be covered 100%.

I hear ya--I just finished my 6-month diet yesterday. It sucks to have to wait, but it should be worth it in the end! And it should give you lots of time to research everything and really plan everything out--I know I could draw you a diagram and write you a book about how and why the DS is right for me, how I will have to eat post-op, what supplements I will need and why, what type of follow-up care I will require, what kinds of complications are most common, who the best DS surgeons in the world are and where they are located, what my surgeon's qualifications are and why I chose him, etc. You can also use these 6 months to get your body into the best shape possible--being physically stronger (think abdominal muscles and cardiovascular health) will make your surgery recovery much easier.
I have the same insurance as you do...bcbs of PA (PPO)....I am on month 5 of my 6 month diet.....I was denied originally w/in about 1.5 weeks because they wanted 6 consecutive months (even though the policy says 6 cumulative months, they still wanted them to be 6 in a row w/in a 2 year period prior to surgery...ug)....but I am actually glad...I will get through the holidays & have my last appt in January w/my PCP....then we don't have to start from scratch, we can just submit the additional proof of physician supervised diet &, hopefully, be done.
