Anyone have Example of PCP Letter "Doc asked for them"

My PCP ask me if I could get some examples of Letter's other Doctors have written. He has never done one and wants to be sure to put in it what  the insurance co will be looking for.

Hi, Dianne. I drafted letters for my PCPC to sign and Aetna accepted them with no problem. I got your email from your other post and will send them to you.
