what is the best insurance to have for garstic bypass or the sleeve?

OK its time to change my insurance plan at work and I would like some advice on what type of plan works the best for weight loss surgery. I currently have united health care high options pos. It seems to have very good coverage but I still have to pay the surgeon fees total of $6000.  what should i make sure is included ?what don't i need?
