I have Humana PPO through the state of Kentucky because of my dads job.  I have been trying to have surgery as well. I was told my Humana PPO that if it is medically necessary then they will approve the surgery. However, they told me that they will not pay for the 6 visits to a doctor for the supervised diet. It is best just to call the customer service number on your card and ask them about the surgery. They are able to give you the most insight into the process that your husband will need to begin. Hope this helps!
