Humana members may be eligible under the Plan for an • Have medical complications of redundant (excessive) tissue such as candidiasis, intertrigo or tissue necrosis (changes in tissue caused by cell death): abdominoplasty or panniculectomy for patients who: AND Are unresponsive to at least 3 months of documented (as evidenced by photographs and medical records), conservative management (oral and/or topical medication); AND For procedures that are being performed following significant weight loss, there should be documented evidence that the patient has maintained a stable weight loss for at least six months.
