Tuffts HMO Cover revisions? or CIP (GIC) in Massachusetts

Does anyone have experience with revisions  with Tuffts HMO, or with Comm Indemnity Plan in Mass.  I need a revision andf can either keep Cobra-ing the CIP or get Tuffs through husbands work.  BMI only 38.6 but big time comorbids
