Has anyone ever heard of an employer not requiring a 6 month supervised visit even if the insurance company does in some plans?

If the insurance company requires you to do a 6 month diet then you have to do a 6 month diet.  I don't know how your employer would have the option to take that away when they feel like it.

Yes, it could be that your employer doesn't require 6 months supervised weight loss but your insurance wants it.  You'll need to find out who wrote your plan and about any special provisions to see who has the final say.  It works one of two ways...your employer writes the plan and has the insurance carrier administer benefits or your employer has the insurance carrier write the plan and administer benefits. The writer of the plan has the guidelines you'll need to follow and is who you'll need to work on for denials.
