Need help w/ behavior modification letter

Hi, I usually don't post but I called my ins. co. today and was told I have everything they need except the behavior modification part of the six month diet. The nurse told me I can get my pcp to write a letter containing the behavior plan. Does anyone have a sample letter or anything? My pcp is a sweetheart but I know he will need an example.Thanks for any help.

PM me with your address and I'll get you copies of the ones I submitted (I typed them up and the doctor signed off on them). I only had to do a 3 month regimen, but my letters should give you a guideline to follow.
