6 month diet Physician Letter (Any Examples?)
Does anyone have an example letter that they wouldn't mind sharing with regards to the letter that your Physician writes after your 6 month diet.
Today is Visit # 3 and the start of Month 3
I was thinking maybe at my last Visit in January I could walk in with a letter and just ask the doctor to read it and sign it for me. That way it's easier on both of us.
Also.. At the close of month 2 I've decided that the whole Weigh****cher deal is a bust. I don't forsee myself spending another 120 a month for something I can't do. (let me explain)
I'm doing weigh****chers at work and the scale they use only goes up to 300 lbs. I have to drive to the next town to weigh in at the corporate center because their scale goes up to 600. To me the gas I'm spending should be deducted from my dues.
Any comments on that? Am I shooting myself in the foot? I might just sign up for the online version and continue going that way.. I mean I love the "plan" but the whole meeting thing doesnt do it for me since I cant partake in the weigh ins

As far as the documented 6 months, my insurance company wanted the actual Dr's notes from those individual 6 months. They would not accept a letter from the Dr...

I submitted office notes and a letter for each visit, showing compliance with the regimen. I only had to do 3 months, but if you'll PM me with you address, I'll get you copies of the letters I drafted and then the doctor signed.
