Does anyone know what they are looking for with 5 years documented. Just the weights in my chart when I have Dr appts ? What if I'm not within the criteria ? Can I appeal and win that ?

My PCP wrote a letter listing my weights for each year.  I also had copies of the pages of OB/GYN wrote my weight on.  This was given to my surgeron, but I don't know how he submitted the information.  I had UHC when I had my surgery and this was fine.  My BMI was over 40, but I didn't have any co-morb. that I was taking meds for.  I had the beginnings of sleep apnea and high blood pressure.  I was approved in 4 days.

Good luck!

I think that they are just looking to see if you have been overweight for 5 years and that you have not had major shifts in your weight.  I did not even go to the doctor every year for the past 5 years (due to lack of insurance coverage) and I was still approved with no problems. Most time your surgeon's office can look at your records and tell you if they are acceptable (expectally if all they do is wls).

I am going through that now.   I don't know why I thought it would be so easy.  I had everything I thought I needed.   Then UHC denied me because they didn't have 5yrs documented.  I had back to back pregnancies and both were high risk due to my high BMI.  So I called my OB/GYN but since I haven't been there in close to 3yrs my files are in storage....and it will take some time to get it.  I am so upset because I want this done like yesterday....

Good luck to you.
