APPEAL FORM???????????

HELP!!!!!!!!!!!!!!!!! I have Cigna ppo and I sent in my second appeal to the second appeal department and I just wrote them a letter and sent new documents that I had.One girl I talked to asked me if I used an appeal form.I was shocked because I didnt even know there was an appeal form.I dont know if I even mentioned appeal but I did state that I had received my denial letter from them in March and told them more info. on why I needed the surgery.Did any one else not use an appeal form?

If it's not personal, can i ask why you were denied? I'm being submitted this week.

I was first denied because I did not have a bmi of over 4o for aleast a year and that my commorbitities were not bad enough.That was in March and my bmi had only been at 40
since last August. I had 180 days to appeal so I waited till this August and got weighed every
few months at the Dr.'s and wrote a letter on my health problems .By the way they approved
me yesterday and now I am scared to death of the surgery-stupid hugh?? Well I hope this helps you.
