requesting a appeal to Cigna

 Boy, I sure hope I haven't messed up but I just sent in my level 2 appeal to Cigna
and I just wrote a letter asking them to approve me and told them more about my
disabilities and sent them my Doctors visits with my weight recorded up till now.
My question - is there a real form that I should have filled out.I thought they said just
send in more information but my denial said to send your request and any new
documentation. Am I still o.k.--- any help will be appreciated.

Did they tell you why you were denied?

They said I was denied because my bmi was 40 starting last August.I waited till this August
(which was still within my 180 days to appeal ) to send in the rest of the years Doctor visits
showing my bmi at 40 or above.They also said my commorbidities were not life threatening but
I am am disabled with arthritis that causes alot of inflammation so I sent them my disability
papers (some are from them were I draw long term disability till I am 65) I told them what
all the inflammation does plus I have fibromyalgia,high blood pressure,high cholesterol, bone
spurs on my knee and stress incontinence,so I hope they approve this time.I only sent new papers because they should have all of the other papers I have sent.

Make sure you send all documentation from the physican's office about your comorbidities and have your PCP document it as well.   Good luck!!
