Any Nebraskans with Carefirst BCBS?

Just wondering if there is anyone who lives in Nebraska who has Carefirst BCBS out of Maryland?

What did they require of you for surgery?

Hi I am not from NE but have BCBS of Maryland. Here is the word for word criteria I got of the BCBS website. Hope it helps I hope to be hearing something back this week from my Dr's office. The paperwork was sent in July 16th so BCBS should be getting back to us sometimes this coming week. Good luck I have been working toward this surgery for the past 4 years.

AGE - 18 years old or older and

Psycological examination - Complete a pyscological examination to determine readiness and fitness for surgery and necessary postoperative lifestyle changes and

Structured diet program - Maryland and D.C. plan members only: must complete a structured diet program in the 2 year period that immediately precedes the request for the surgery by participation in either;

- one structured diet program for 6 consecutive months or
- two structured diet programs for 3 consecutive months. (can include commonly available diet programs such as Weigh****chers or Jenny Craig)  and

Weight Requirment - Maryland and D.C. members only: must meet either of the following criteria:
- BMI of 40; or
- BMI equal to or greater than 35, in combination with one of more of the following co-mord conditions: hypertension, a cardiopulmonar condition, sleep apnea, diabetes mellitus, or any life threatening or serious mdical condition that is weight induced.

Thanks for your response.  My insurance did get back to me and say I needed 6 month medically supervised diet and a psych eval.  I was aware of the psych eval but bummed about the 6 month supervised diet.  Oh well, the hoops we have to jump through I guess.  Thanks Again,
