complications: denied by ins??

I had the rny in 04, approved without any problems.  Had lap in 05 for tear and internal hernia. Ins no prob.  I've been having problems ... had upper gi and found the pouch is of normal size, hiatal hernia, and stoma is wide open, with free flow.  MY insurance is refusing to pay, saying is is an exclusion.  What do I do ?

I did call the hr and she said she will work on it... so? any suggestions?

I would ask your question on the revision board, too, that board is a bit more active than this one.
