Question About Oxford

I've been lurking here for awhile reading up on everyones experiences and preparing for my own (hopeful) surgery. I have a question about Oxford (Freedom if it matters).
Everything I've read from other surgery patients has had nothing but wonderful things to say about how easy it was to work with Oxford. When I spoke with them (and later looked it up on their website) they said they require a BMI over 40 and a letter of medical necessity. No supervised diet or anything else. My doctor's office however is telling me that I need a five year history of morbid obesity. That would be fine except I'm missing 2004 (I do have 2003 though).
I'm not sure if I really need this documentation. The sheet of requirements they gave me says it's for "Oxford & United Health Care". I think the five years might just be for UHC and I've read a lot of mixed reviews about that insurance.
Has anyone had experience with Oxford and the five year history? I've actually been losing sleep over this (literally) so any guidance would be much appreciated.
