Is there a sample letter for DH's Company?

DH company also has a self-funded insurance plan, with Americas TPA as the administrators and First Health as the Network.

First Health covers WLS, but DH's company has an exclusion on it. When my DH called to see if it was covered the rep told him no but that they have had a lot of requests for it and may discuss removing the exclusion during the new open enrollment in October.

I would really appreciate a sample letter to send to the company's Benefits Manager and anyone else you can think of to help them understand how important this is.

I am only 26 and suffering with depression, sleep apnea, anxiety, and arthritis in my right knee.
