Any one appeal themselves?

My new surgeons office is refusing to send in my appeal stating that my office visits with my PCP do not count towards my physician assisted weightloss requirements even though Cigna instructed me that after my 3 nut visits i needed 3 additional visits with my PCP.  Has anyone out there went on their own to do their appeal?  I am calling my case manager from Cigna tomorrow to make sure that it all counts.  I've been rescheduled twice already and now my surgeon left the practice so I hope that doesn' t hurt my chances of going to someone else.

I  have noticed that you provided your asssitance to help people with appeals.i just got a deniel letter from my insurance company threw my employer United Heathcare (employer - Time Warner Cable) for a revision from the Lap-Band  to the gastric sleeve. The reason was "Investigational"
