My (PCP) Dr. seems to have lost my records. He moved offices, they might be in storage(long story) The thing is I don't go to the doctor very often. It might have been more then 5 years between visits. I started going again last September I have high blood pressure, sleep apnea, and a BMI of over 50. I have done the 6 month diet and all the tests my surgeon asked for.  Be fore he submits the claim he wants my medical records for 3/5 years. What if there isn't any except for September 2007 on? Any opinions on if this will be  a problem or not? Will I be denied if I haven't gone to the Doctor for the first four of the last five years?

I don't think you need 5 years of medical records.  What seems to be the rule for insurance purposes is a 5 year weight history... just your weight for the past 5 years to establish you've been morbidly obese for a while. Some insurances ask only the past 2 years, but most ask 5 years. You might want to call your surgeons' insurance person to clarify if they want your entire medical history or just one weight per year for the past 5 years.
