Premera BCBS Microsoft PPO plan - anyone have knowledge

I have this plan and it looks like they have alot of loops you have to jump through before considered for bariatric surgery.  Premera puts this together with Microsoft.  They have like 5 levels of weight management programs you must complete and fail in order to get to level 5 and be considered for surgery.  Anyone out there have this plan and was successful or not?  Thanks so much in advance.
