Any info on Empire Blue

I have Empire BCBS and wondering what the criteria is.  It is a covered benefit, but need to know what they require.  thanks!

Hi I know that Empire is one of the insurers that NYU Program for Surgical Weightloss is familiar with and they are in that plan. I believe they have a 3-6 month medical weightloss requirement. However some companies have riders to their specific coverage and you will need to get in touch with Empire directly to find out if your company has any exclusions.   I suggest calling NYU and asking what their experience has been on the reqs. 212.263.3166.  There surgeons are very experienced and are highly regarded.  I will be getting my band done there but I have to pay for it out of pocket.  They have a outpatient option that is about 17-18K.
