Which insurance covers Bariatric surgery right now - hurry!!!

In the states of Ohio, Texas, or Arizona. I'm making a case for my HR department to switch health insurers for next year and need to know who actually does cover it. We currently have Anthem BC/BS and they are giving me the runaround, blaming it on my employer, but I think they don't cover it at all for anyone.
Duodenal Switch hybrid due to complications.

I have put together quite a few Anthem preauthorizations that were approved.  It depends on the employer.  IF the employer wants it as a benefit then they purchase the product with that included.  If it is a self funded plan than the insurance company adminstrates the plan and it goes by what the employer puts in their certificate of coverage.  Either way, the employer is in the drivers seat.  Good luck, you are doing a very worthwhile thing. Larissa Anthem does offer WLS riders on policy for employers with groups of 100 or more.  It is not Anthem who makes the decision whether a company has WLS for their employees.  The employers make the ultimate decision.  But Anthem does have a medical policy in place and does cover WLS on selected contracts. Humana, UHC, Aetna, Anthem, all have WLS policies just have to convince your employer to take the WLS rider
