AvMed - Fl State employee * Help!!

If anyone knows the answer to this, please post!  I have AvMed Florida State Employee insurance.  WLS is a covered benefit, but that's all I could get out of them.  I asked them for the criteria for a approval and they WOULD NOT TELL ME!!! They said that I would have to ask my surgeon, and that THEY would know..  My surgeon said it was MY assignment to figure out what was required...  Boy, am I in a fix.   I know they require that '6 month supervised diet' thing, but I don't know just exactly what has to be written for each month.  My PCP doesn't know either.  I've heard horror stories about how people have complied, but say in the second month, they also had a cold...they made them start all over.  Also, if the exercise is not documented...Just a bunch of things.  I know that in CA there are appeals that have been documented stating that the 6 month diet does not work, and people have won the appeals from just that point.  But...I don't live in CA.(other end of the world, actually).  Basically, has ANYONE been successful with AvMed insurance covering your WLS, and WHAT were the requirements?  Thanks in advance for your time!
