Bonnie or anyone Help!!!!!

My DD is wanting RNY she has BCBS of Tn. Do you know what all they need?  And if she needs 5yr stuff because she has nothing, we moved from up north to Tn. and got reed of alot of stuff. so if you could help I would be so Thankful.        Thank You Sue

Yes, you need copies, if there is nothing around create a diary or list of what you did during that time, because you really only need to prove this was your last resort.
