meritain health of minnepolis

I will soon be on meritain health insurance of minnepolis and I was curious to know if anyone has this insurance and if they cover lap band surgery and if its a hard process or if its pretty easy to get approved for lap band.  I am 5foot 6 and weight close to 230 lbs.  Appreciate any info!
