Your BMI has to have been above 40 for 5 years, unless you have a UHC policy that allows BMI's from 35-39 with co-morbid conditions like diabetes. You only need one weight per year that shows you had a BMI above 40, so choose your highest weights.

It depends on the policy!
If you have a Medicare policy through UHC, then you DO NOT have to deal with the 5 year weight history thing. If you have a policy through an employer, than you do. There are also differences between the individual policies, some only cover surgery if your BMI is above 40 period, no coverage for a BMI below 40 no matter if you have diabetes, high blood pressure, etc. UHC policies vary greatly.
