Guardian One Net PPO

I have a date for July 2nd for a revision. I had lap-band about two years ago and lost about 50 pounds but once I started back eating soild food I gained all the weight back except 7 pounds. So my surgeron agreed that we should try some thing else so I am going to have the sleeve done. My question is when I had my first surgery my company had Guardian Alliance PPO now they have changes to Guardian One Net PPO. For some reason when I called them they told me that they didn't have a record of me having Lap-band surgery. Does anyone think that it will be a problem with my getting approval for a revision. Is it all about they way the surgeron writes up the claim. If there is something else that I need to do please let me know. I have just came the the realization that I really need help losing weight and I think that this is my last hope so I really want to get this approved. Thank you for all the help. Chastity
