I'm a nurse for Dr. Boyce in Knoxville, I deal with UHC all the time. Your requirements depend on which employer the policy is from. They all require 5 years of "morbid obesity" and all require a psych eval. Some require your BMI be above 40, no matter what, others accept a BMI of above 35 if you have co-morbid consitions like diabetes. Some require a 6 month supervised diet, some do not. You will have to call them for your specific policy info.  You might have to talk to 3 people to find out the information, just keep asking for the exact crtiteria. If they say BMI above 40 and do not mention BMI above 35, you know your BMI must have been above 40 for the last 5 years. If they do not mention diet, you probably do not have to do one. Call them at least twice to make sure you heard it all right. Oh, I just saw you have a band. Their policy is you have to have a problem with the band, like it slipped, or other complication, for them to pay for conversion to RNY. They will not pay for it simply because you did not lose enough weight. Bonnie

I've had problems with my band from the beginning...its not because I didnt lose enough weight..I did..but its been unfilled due to problems at least 90% of them time I've had it and during that time I've gained even more weight, its been a roller coaster ride.. I have an appt wednesday to see about the rny so we'll see what they say..I was told bmi of 35 if I have comorbidities which I am diabetic, high blood pressure and a few other problems...hopefully it works out for me....thanks!
