BCBS Lumenos?

Has anyone ever heard of them or dealt with them?  Do you know what they require?  Any info will be greatly appreciated.

Lumenos criteria is very similiar to BC/BS Anthem.  Psych eval, weight loss documentation, Detailed past diet attempts, Your primary MD putting in writing that you are cleared for surgery.  They usually approve if you have these things.   Mary

Thanks Mary.  I work for Charter Communications(again) and have Lumenos.  I know Charter doesn't cover it, but I'm thinnking of trying anyway.
