Pacificare CA HMO?
The state by state insurance co list isn't working right now...so I'm hoping someone will have info for me. My DH is changing jobs and we need to make a decision ASAP about which ins co to use.
We have the option of Pacificare HMO or Kaiser (our current).
I'm hoping to have Lapband, BMI of 47, no comorbidities, no history of supervised weight loss. With Kaiser approval would be difficult/ unlikely- since they say they require comorbidities under 50 BMI. And I would need to go through 6 month supervised diet, then followed by their 6 month Bariatric program.
I assume that pacificare will be easier...but I'd love some input.
Anyone know :
BMI requirements?
6 month supervised requirements?
Open to lab band?
Any approval difficulties?
Any help or info would be greatly appriciated!
Thanks in advance,
Eris
My company has the same choices for health insurance. I had chose pacificare because the doctos are close to my home. I am currentlyin their bariatric program right now. You get a referral from your primary- go to an endocronlogist- he or she sets you up with the 6 month nutritional class you need to lose approximately 10% of your body weight then you are aprroved for the next step. I am just about there right now scheduled to meet with the doctor on the 5th of May for my autorization to head to the next step when I find out exactly what that is I'll let you know but either way it isn't pretty. Alot of hoops to jump through. Hope this helps in your decision.
