5 yr history

hi,  I have my 6 month diet taken care of and my pych eval. All that is needed is a 5yr history, the 3 doctors that should have it no longer do -due to some kind of office damage. I did find I donated blood exactly 5 yrs ago, but the blood center is giving me the run around. I 1st verified they had my records which they do, but now they're saying they're in storage and the director has had surgery and is out. Any advice would really be appreciated.

Sorry, but you need it. I have not been able to get around any insurance company that requires it. Make sure you think, think, think! It does not matter where the records come from--did you go to weigh****chers in 2004, etc??? You can contact them and get a letter stating your weight. What about LA weight loss, your ob/gyn's office, a walk-in care center, etc..   Your dentist can write a letter for you stating although he did not weigh you that you were obviously morbidly obese. If your doctors lost your records, force them to write a letter on your behalf stating what happened to the records. You can find them somewhere. Good luck! Bonnie

i had only 3 recent yrs.2008-2007-2005-. I went back to 1997. so go further back. it does not say "consecutive" yrs does it? use what ever you can find. let me know if you need more help.
